TERMS and CONDITIONS: Str8-4ward Training Concepts LLC reserves the right to charge a $25.00 fee if payment is not received one week prior to the scheduled training date. Class reservations may be cancelled up to 7 calendar days prior to the first day of class with a 100% refund, less than 7 days, but prior to the first day of class with a 50% refund, on the first day of class and ANY “no shows” with a 0% refund. If the scheduled training is cancelled by Str8-4ward Training Concepts, we will refund your payment 100% and we will be held harmless. Agencies may substitute or replace an enrolled student within 24 hours of the scheduled training. I acknowledge & agree to hold harmless Str8-4ward Training Concepts LLC for the above listed training course. A 3.8% credit card service fee will apply to the invoice.
